Contracting and Management

ACN Group's goal is to work collaboratively with health care professionals who contract with us, providing them with the information they need to provide quality, cost-effective care. Through communication and data sharing, we work with health care professionals to ensure patients get the right care, from the right provider, at the right time.

Data Sharing
Health care professionals are given quarterly and annual information regarding their clinical performance. This information gives them an opportunity to review their individual performance, recognize achievements, and identify challenges.

One-on-One Attention
In addition to our wealth of educational resources, our support clinicians work one-on-one with health care professionals to promote best practice care.
